A Parent's Guide to UIL Sports Physicals in Texas

If you've got a middle or high schooler playing a UIL sport this year, there's a form standing between them and the first day of practice. Every athlete in Texas public schools needs a pre-participation physical on file before they can step onto the field. It's not a formality the school invented. It's a University Interscholastic League requirement, and coaches can't let a kid participate without it.
The good news is that the physical is quick, straightforward, and cheap. Here's what you need to know to get it handled without stress.
What a UIL Sports Physical Actually Is
The UIL pre-participation physical is a focused checkup that answers one question: is this kid healthy enough to safely play a competitive sport? It's not a full annual wellness exam, and it's not a substitute for one. It's a targeted screen for the things that matter on a field or a court.
Texas requires it before the first practice, and it has to be renewed every year. A physical from last season doesn't roll over. The UIL updates its rules periodically, so the current year's form is the one that counts.
The Form You Need
The paperwork is the part parents get tripped up on, so let's be clear about it. The UIL uses a specific Pre-Participation Physical Evaluation form, usually called the PPE. It has two main parts.
The first part is the medical history, and you fill that out at home with your athlete before the appointment. It asks about past injuries, family heart history, breathing issues, concussions, and anything else a doctor should know before clearing your kid. Take your time with it. An honest, complete history is what makes the exam actually useful.
The second part is the physical exam itself, which the doctor completes and signs at the appointment. Both parts get turned in to the school together. Your school or coach will tell you where the completed form goes, and many districts want it uploaded to an online system like Rank One. Bring the form with you, filled out on the history side, and you're set.
What the Doctor Checks
The exam moves through the basics that keep a young athlete safe. Height, weight, blood pressure, and pulse. A look at the heart and lungs, listening for anything that sounds off. Vision. A check of the abdomen. And a musculoskeletal screen, where the doctor looks at how the joints move, how the spine sits, and whether there's any old injury that hasn't fully healed.

When to Get It Done
Here's the single most useful piece of advice in this whole post: don't wait until August.
August practice starts fast, and every parent in your district is trying to get the same form signed in the same two weeks. Clinics fill up, and a kid who doesn't have a cleared physical on file simply can't practice, even if they show up ready to go. Missing the first days of practice is a rough way to start a season.
Late July is the sweet spot. Get it done now, turn the form in, and your athlete walks into day one already cleared. If something on the exam needs a closer look, you've also given yourself time to sort it out before it costs them field time.
